Vax Clinic

Dear Families of Students Ages 16 and Older:

Rite Aid of Ada has offered to share doses of the Pfizer Covid vaccine with Ada Schools students, and to host two clinics, one for the first dose and one for the second dose, on our campus.  The first clinic will be THIS Thursday, May 6, from 8:30-10:30 A.M. in the 86 gym.  The second dose clinic will be on Thursday, May 27, from 8:30-10:30 A.M., also in the 86 gym.  The Pfizer vaccine requires two doses, spaced 3 weeks apart.

Students who wish to get the vaccine at these clinics will need to have the form filled out and signed by a parent (to give consent/permission), and to bring an insurance card (if they have one) and a photo I.D. (driver license).  If a student has insurance, Rite Aid must bill the insurance company: however, if the student has no insurance, or if their insurance does not pay for the vaccine, there is NO COST for the vaccine.  The student does not need to bring any money and you will not get a bill for the shots.  If students do not have a Photo I.D. please let us know, and we can print their DASL screen with picture to verify their identity.

Below are links to the form that needs filled out, and two informational documents, one about the vaccine and one about an app that helps track side effects and symptoms (and is optional).  In order to let the pharmacy know how many doses to bring with them, parents of any student who is planning to get the vaccine here at school, should fill out the Google form, to which there is a link at the bottom of this letter as well.

On the day of vaccination, students will be called to the gym by last name (students getting the vaccine whose last name begins with A-E, then F-J, etc.), so they are staggered.  Seniors who get the first dose on the 6th, should report on the 27th directly to the gym at 8:30, and will be the first group to get their shots that day.
